As an applicant for CHARM membership, I fully understand and agree with the following requirements for membership  However, if, at any point, I fail to abide by these requirements, my membership will be considered invalid.

1. I am ultimately responsible for my children.  My spouse or I agree to be present at all CHARM activities our child attends and, thereby, take full responsibility for their safety and actions.  In addition, I will not hold any CHARM member or leader responsible, or legally liable, as we together serve the needs of our children.
   
2. I agree to provide health insurance or be financially responsible for medical care in the event of injury or medical emergency. In the event I am not immediately available, I give permission to initiate emergency medical care to my child for injuries or illnesses.
   
3. I will faithfully serve on at least one committee of CHARM this year.
   
4. I will abide by all laws, as they currently exist, governing the operation of home education in North Carolina .
   
5. I will abide by the policies set forth in the Handbook governing our group.
